waIT!... I NEED A BIT MORE INFORMATION ABOUT THE PERSON WHO WILL RECEIVE THIS SOUL PLAN MINI READING!

Below is a form for you to complete.

NOTE: For this Mini Reading I need your full birth name as stated on your birth certificate (first and last name). This means all your first names and your last name at birth (i.e. your maiden name and possible baptismal names). Your Soul Plan Mini Reading is calculated based on your full birth name. So it is very important that this is complete and correct. Please also pay attention to typo’s!

I prepare your Soul Plan Mini Reading personally and thoroughly. Then I’ll email your Mini Reading in PDF format. If, after you received your Mini Reading, it appears that the birth name you supplied is not correct, then I can no longer correct it and the Mini Reading will no longer be appropriate for you. That means you will have to order a new Mini Reading.

  • Please complete the form below with information about the person who will receive the Soul Plan Mini Reading (either yourself or someone to whom you will give the Mini Reading as a gift). Be sure to enter the full birth names (all first names and birth surname(s) as listed on the birth certificate). Also fill in whether you would like to receive the Mini Reading in Dutch or English.
  • Upon receipt of your information, I will prepare your Mini Reading as soon as possible (no later than 5 business days) and send it in PDF format to the E-mail address provided in the form below.
